Administrator: Meagan Buckley-Leonard, NHA

Meagan attended University of New Hampshire where she received her B.S. Degree in Health Management and Policy with a focus in long-term care. Her experience has varied from Continuing Care Retirement Communities, to Assisted Living Facilities and she by far enjoys working in Skilled Nursing Facilities. Meagan has a passion for serving the elderly and the community. While not working, Meagan spends her time with her 2 energetic dogs, Maddie and Ivan.

Director of Admissions: Kathy Walker, LPN

Kathy joined Berlin Health and Rehab in 2007 as the Admission Coordinator. She is a Licensed Practical Nurse and has an Associate Degree in long term care administration. Kathy has worked in long-term care for over 30 years and has a strong commitment to the elderly. She can assist you and your family with any questions you may have about coming to Berlin Health and Rehab for short-term rehab, respite or long-term care.

Director of Nursing Services: Chris Malone, RN C.

Chris served in the United States Air Force as a survival instructor for four years before starting his nursing career. He assumed many roles from NA, LPN to RN from Medical Surgical to Pediatrics and finally Long term care. Chris attended Herkimer County BOCES where he received his LPN and then later received his A.S.S. Degree from Regents College of the State of New York. Chris lead Berlin Health and Rehab towards obtaining its Joint Commission gold seal rating.

Rehabilitation Director: Jason Revoir, MS CCC-SLP

Jason earned his BS Degree from the University of Vermont and his Masters from the University of Rhode Island. Prior to coming to Berlin Health and Rehab Center he worked in Northern Vermont in a variety of settings from young children to geriatrics. He especially enjoys working with people with acquired language and cognitive disorders and helping them return home. Jason enjoys playing soccer, sailing, skiing and being with his family. Jason lives in Shelburne with his wife Jenn and son Ethan.

Director of Resident & Family Services: Heather Filonow, MSW

Heather joined Berlin Health and Rehabilitation Center in 2005 as the Director of Resident and Family Services. Heather holds a BA in Psychology from Trinity College of Vermont and a Master's Degree in Social Work from the University of Vermont. She has over 10 years experience working in a variety of settings and with diverse populations. Heather practices from a strengths perspective and is versatile in working with individuals, families, and groups to support them with resources to meet their respective goals.

Activity Director: Deborah A. Matt, N.A.A.P., N.C.A.P.

Deb brings over 10 years of experience in activities, social services and admissions. She is a hands on professional that works closely with her team to provide the best environment and quality activities. She strives to ensure all activities are meaningful and have purpose for all. Deb is married with 3 children and 1 dog, in her spare time she is a hospice volunteer and enjoys the outdoors and time with her family.

Business Office Manager: Kelly Royce

Kelly has 15 years of Office Management experience. She is always available to assist families with their financial questions and challenges. Kelly recently moved to the Berlin area from Southern New Hampshire where she had lived her entire life. She thinks Berlin Health & Rehab Center is a beautifully located home and enjoys her job and all the people she has the pleasure of working with.
